We exist to close gaps in access, reduce inequities, and help programs thrive, not just survive. Through shared infrastructure and collaborative support, we're strengthening how health and community partners deliver care across the islands.
What We Do
From our roots as an Umbrella Hub supporting the National Diabetes Prevention Program, we've evolved into a statewide connector linking payers, providers, and community-based organizations. We provide:
Administrative & financial infrastructure for sustainable community health programs
Food Is Medicine program development and support helping health centers connect patients with healthy, local food
Coordinated support for quality improvement, billing, and impact reporting
Innovative care models including Age-Friendly Health Systems
